For a variety of reasons we require all patients to have a valid form of ID on file. Upon intake you will be asked to upload the front and back of your valid government issued ID. There are no exceptions to this policy. If you do not have a valid government ID for your chart, we will be unable to see you. This policy is for all visit types, whether in person or telemedicine.
PPO plans, We are currently in network with United Healthcare, Aetna. and Cigna.

Although we run verification of benefits, often times this can take some time to come back into your chart. If you overpay for your visit and insurance has returned an EOB, if a refund for any portion of your paid visit is due, you will receive a credit or refund back to your payment method on file.
All patients are required to have a payment method on file. Upon your intake you will receive an authorization form. Appointments are paid for and charged on the day of the visit. If you need to update your payment method please contact our office here. If you are a current patient and have not been switched over to our new system, you may receive a request to fill out an authorization form. Please be sure this is done prior to your next appointment to avoid any delay in seeing your provider.
Any patient on a controlled substance requires a 30 day follow up to be seen by their provider. We do not send in refills for any controlled substance without a follow up face to face with your doctor or provider. There are no exceptions to this policy. Non-controlled substances may be refilled depending upon your treatment plan and at the discretion of your provider. Please discuss this with your provider during your visit.
All patients receiving a controlled substance are required to sign the controlled substance agreement. The controlled substance agreement will be kept in your chart. When signing the controlled substance agreement you are agreeing to our practice policy. There are no exceptions to this policy. For minors, legal guardians or parents must sign this contract and be kept on file in the patients chart. Please also note, that if you do not have a valid ID on file, no controlled substance will be sent in until you have updated our office with your valid ID.
We encourage our patients to check with their preferred pharmacy first if there is a shortage of medication in your area. High demand medications can often have an effect on the providers ability to send in prescriptions. If you are experiencing difficulty finding a pharmacy that can fill your specific medication please contact our office and we will do our best to assist you. Under some circumstances we can re-route your medication to a new pharmacy by sending a request through our messaging portal here.
Most of our medical services are offered via telehealth. If you prefer an in person visit, you must contact our office to ensure we are located in your area. Botox, Fillers and other cosmetic treatments are by appointment only.
Yes, you can. If you are seeking a new provider, you can transfer your care to us. Simply make an appointment, and we will request your medical records from your previous provider or if you have them available you can submit them through our secure messaging portal.
We understand things happen. Our practice charges a no show fee of $50.00 as well as a late cancellation fee if you cancel within four hours of your scheduled appointment. Please contact our office if you have an emergency and we will accommodate you as best we can. Out of respect for our providers please cancel 4 hours prior to your appointment so that another patient can fill that slot.
